Position Available

Patient Navigator

The Patient Navigator will support patients throughout their healthcare journey by coordinating referrals, appointments, follow-up care, and patient education. The successful candidate will work closely with healthcare providers, community health workers, and patients to improve treatment outcomes and eliminate barriers to healthcare access.

Key Responsibilities

Successful candidates will be expected to:
